Eden Clinic Plymouth

How your details are used when you enquire or book online

Who is responsible for your details

Eden Clinic Plymouth is the data controller for the details you enter on these booking pages — it decides why and how they are used. The SyHive platform (operated by ASJS Medical Systems Ltd) processes your details on the clinic's behalf, as its data processor, and stores them securely on the clinic's own records system.

What we collect

The booking and enquiry forms ask for:

  • First name
  • Last name
  • Date of birth
  • Sex
  • Email
  • Home postcode
  • Phone (optional)
  • Message (optional)

Why we can process your details

We process the details you provide because they are necessary to take steps, at your request, before entering into a care arrangement with the clinic (UK GDPR Article 6(1)(b)). Where your enquiry relates to your health, that information is processed for the provision of healthcare under Article 9(2)(h), by or under the responsibility of professionals subject to a duty of confidentiality.

Who sees your details

The clinic's staff, to handle your enquiry or booking. Your details are not used for marketing and are not shared with anyone else.

How long we keep them

If your enquiry doesn't lead to registration with the clinic, we delete the details you entered after 30 days. If you register, your details become part of your clinic record and are kept in line with the clinic's health-records retention policy.

Your rights

You can ask the clinic for a copy of the details it holds about you, ask for them to be corrected, or ask for them to be deleted. If you are unhappy with how your details have been handled, you can complain to the Information Commissioner's Office (ICO) at ico.org.uk.

Contact

For anything about your details, contact Eden Clinic Plymouth directly — it is the controller of your information. Questions about how the SyHive platform processes data on the clinic's behalf can be raised with ASJS Medical Systems Ltd's Data Protection Officer, via the clinic.
